About the Medical
The DVLA sets the medical standards for Group 2 driving licence
entitlement. There is some useful information that you can download here
that outlines some of the important points of the medical at the
surgery.
A D4 medical form will need to be filled out by the doctor at the time of
your examination. You can download a D4 form
here, although we do have plenty of them at the surgery. There is a very
useful explanatory leaflet about the D4 form called INFD4 which can be
downloaded here.
IMPORTANT POINTS TO REMEMBER ABOUT THE MEDICAL AT THE
SURGERY:
- THE PRICE FOR AN LGV/PCV MEDICAL IS CURRENTLY: £50. PRICES ARE
SUBJECT TO CHANGE SO DOUBLE CHECK THE PRICE WHEN MAKING YOUR
APPOINTMENT.
- PHOTO IDENTIFICATION MUST BE PRODUCED AT THE TIME OF THE
MEDICAL EXAMINATION. (e.g. Passport or Driving Licence) OTHERWISE THE MEDICAL
WILL NOT PROCEED - NO I.D., NO MEDICAL!
- Payment needs to be by cash or cheque only. As yet, we do not have
card (Switch/Visa etc) facilities. We are looking into setting up a
Paypal account to deal with credit card requests.
- We expect 24 hour notice for cancellations.
- If you wear contact lenses or glasses please bring them with you. If you
wear contacts you will need to take them out during the examination as your
eyesight will need to be checked with and without them in.
- Please bring with you a list of regular prescribed medication taken -
ideally a copy of your 'repeat prescription' slip.
- If you are currently under the care of a Hospital Specialist or
Consultant, please bring the Specialist's name, contact details and your own
hospital number with you.
- Be prepared to pass a urine specimen at the time of your
examination.
